The cost of diabetes currently exceeds $1 trillion annually and is estimated to rise above $2 trillion by 2030. This amounts to more than 2% of global GDP. One in every 11 adults has diabetes with more than half of those undiagnosed. The crisis in diabetes healthcare is not a crisis of strategy or clinical knowledge; it is a crisis of execution. The medication, guidelines, procedures and processes exist. What is missing is the last mile of execution, helping people with diabetes to understand and adopt the medication and behaviours that will lead to sustained and positive change.

The complexity of managing diabetes means that what happens in a clinical setting does not necessarily translate to what happens in reality. Treatment of diabetes is complex and human behaviour plays a big role in outcomes. This has a commercial impact on insurance companies and drug and device makers. Insurers see huge overruns on hospitalisation costs, which hurts their bottom line More than 10% of total healthcare claims are from people with diabetes. Further, hospitalisations of people suffering from diabetes are 30-50% higher than they should be. Premature mortality from diabetes is also on the rise.
For drug and device manufacturers, the real-world results do not match what they see in clinical trials. This results in low uptake rates of treatments that work in a research setting. Further, there is poor ongoing usage, even after patients start treatment. Poor results make insurers hesitant to fund products and this impacts pricing and the revenue growth of these companies.

Our coaches are the missing link in managing diabetes. Through diabetes coaching we have demonstrated net benefits to insurers, drug and device makers, and people with diabetes. With Guidepost, the diabetic patient is better able to manage their condition, go to hospital less frequently and live longer, healthier lives. People who join Guidepost reduce their HbA1c (a measure of blood glucose control) by 2.5 percentage points (with larger reductions in high-risk groups such as those new to insulin and post hospitalisation cohorts), this significantly reduces their risk of diabetes related deaths and heart attacks. People on our programme have a 19% lower chance in being hospitalised, and live longer. This benefits medical and life insurers.
